Eagle Point Dental Scholarship
Funded through donations and a reciprocity agreement of donated services between the Eagle Point Dental Office of Dr. Keith Ogawa and Dr. Rania Haddad and the D-9 Foundation.
Award Annually the Eagle Point High School Scholarship Committee will review applicants and determine the recipients of these awards. The number of awards will be based upon a memo of understanding between Eagle Point Dental and the D-9 Foundation. The actual number of awards will be given to the Eagle Center Scholarship Chair in January so that students will know the exact number of scholarships that will be available. (Two $1,000 awards for 2007)
To Study An approved program of study in any of the Dental Health fields will receive top priority. If there are no students who qualify with the dental other health studies or technologies will be considered. The award is only to be given to the above qualifying students.
To Attend Any Accredited Technical School, Community College, Four Year College or University. Must have federal Title IV designation.
Open To Must be a graduating senior or recipient of GED from Eagle Point High School
Selection Based Upon Merit, class room and related performance, grade point average, involvement in school and volunteer activities, work experience, and financial need may be used as selection criteria by the scholarship committee.
